Why equipment fail?
During our Seminar conducted by Monark Equipment, causes of failure were quantified according to the maintenance practices that most of us failed. As we hear much about empowering self-directed people, we fail to do the correct operating procedure & maintenance practice. I call this over confidence which I believe could lead to an idea that anybody can do their own thing as long as they do those things with good intentions.
The following is the percentage of causes as Monark investigates;
28% Improper Operation
18% Insufficient Lube Oil & Greasing
12% Untimely Change of Filter
11% Improper Water Drain in Fuel Tank
11% Improper Adjustment
8% Insufficient cleaning of Air Filters
7% Insufficient Coolant
5% Poor battery maintenance
